We are looking for a care coordinator to join our experienced care team, based in Amersham, Buckinghamshire. In this role you can have a positive impact on the lives of others, ensuring the best possible outcomes for our clients through effective client carer matching. To be a successful care coordinator you’ll need a methodical approach to problem solving, excellent organisational skills and you’ll enjoy connecting and engaging with others.

About the role:
You'll manage live-in care placements, coordinating rotas, holidays, and any periods of absence. Building strong relationships with carers (new and existing), clients, and families, you’ll ensure the best possible care through effective matching. Accuracy and good IT skills are essential for maintaining our scheduling system and records.
About you:
- Experience in care coordination, scheduling, or similar
- Strong communication and organisational skills
- Compassionate, proactive, and solution-focused
- Confident using IT systems (MS Office, care software)
- Knowledge of CQC standards
- NVQ Level 3 in Health & Social Care (or working towards it) preferred
- Driving licence and access to a car desirable

Together, we will guide and support you to develop your skills and confidence in providing high-quality care for clients, through the Induction and refreshers program, which consists of our CPD accredited online training paired with our experience lead classroom learning.
