The role:

As Care Coordinator, you will have a direct, positive impact on the lives of others. You won’t just be stuck behind a desk, this is a dynamic blend of office management and hands-on field support. Beyond managing efficient rotas, you will regularly visit carers whilst they are supporting our clients to observe competencies, uphold standards, and offer practical guidance. We're passionate about developing our teams and over time, your role will also expand to include carrying out care assessments for new and existing clients.

Key responsibilities:

• Coordinate visiting and live-in care rotas, manage holidays, and cover short-term absence, ensuring carers have sensible, manageable schedules that minimise travel time.

• Work closely with our live-in care team to assist with coordination, scheduling, and day-to-day operational support.

• Conduct regular on-site visits to observe carer competency, carry out spot checks, and provide practical mentoring and supervisions.

• Visit clients to gather feedback and update care plans. As you settle into the role, you will also take on conducting initial client assessments and reviews.

• Maintain clear, warm lines of communication across our visiting care, live-in care, and office teams.

• Maintain accurate record-keeping and participate in the management on-call rota as needed.

About you:

To be a successful Care Coordinator, you need a methodical approach to problem-solving, excellent organisational skills, and a genuine enjoyment of connecting with people out in the community.

Essential experience & skills:

• Prior experience in care coordination within a domiciliary care setting, as a care supervisor, or a similar leadership role.

• A minimum of 2 years' experience in care, with a strong understanding of quality care practices (experience with Dementia/Alzheimer's is essential).

• Strong communication skills, confident use of IT systems (MS Office, care management software), and a good working knowledge of CQC standards.

• NVQ Level 3 in Health & Social Care (or working towards it) is preferred.

• Compassionate, proactive, solution-focused, and flexible to support at weekends and evenings when on-call.

• A valid driving licence and access to your own vehicle are essential for visiting clients and carers in the field.
