The role:
As care coordinator, you will have a direct, positive impact on the lives of others. You won’t just be stuck behind a desk. You will be based both in the office and out in the field, helping ensure the best possible outcomes for our clients by guiding and mentoring our carers, reviewing care plans, and gaining valuable feedback directly from the families we support.
Key responsibilities:
Supporting carers: Guide, mentor, and support our care team through supervisions, spot checks, and ongoing guidance
Client engagement: Visit clients gather feedback update care plans and conduct risk assessments
Team communication: Maintain clear lines of communication within the team
Scheduling: Coordinate rotas, manage holidays, and handle any periods of absence, ensuring our carers have sensible schedules that minimises travel time
Compliance & quality: Maintain accurate record-keeping and participate in on-call duties as needed
About you:
To be a successful Care Coordinator, you need a methodical approach to problem-solving, excellent organisational skills and a genuine enjoyment of connecting and engaging with others.
Essential experience & skills:
Prior experience in care coordination within a domiciliary care setting, as a care supervisor, or a similar leadership role
A minimum of 2 years' experience in care, with a strong understanding of quality care practices (experience with Dementia/Alzheimer's is essential)
Strong communication, confident use of IT systems (MS Office, care software), and a good working knowledge of CQC standards
NVQ Level 3 in Health & Social Care (or working towards it) is preferred
Compassionate, proactive, solution-focused, and flexible to support at weekends and evenings when on-call
A valid driving license and access to your own car are essential, for client visits

Together, we will guide and support you to develop your skills and confidence in providing high-quality care for clients, through the Induction and refreshers program, which consists of our CPD accredited online training paired with our experience lead classroom learning.
